CMS Caps Facility-Level Outlier Payments for Inpatient Psychiatric Facilities

As part of the Centers for Medicare and Medicaid’s final rule for inpatient psychiatric facilities, Medicare will cap outlier payments, which are extra payments for exceptionally costly cases, beginning in fiscal year 2028. Under the new rule, facilities with 50 or more stays per year will have their outlier payments capped at 20% of that facility’s total Medicare IPF payments for the year. Smaller providers with fewer than 50 stays per year will be exempt from the new rule. CMS will increase its payments to inpatient psychiatric facilities by 2.3% in 2027, estimating a total increase of $60 million in additional Medicare payments compared with 2026.
