Colorado-based Canyon Home Care & Hospice has acquired Columbine Poudre Home Care and its affiliate Bloom at Home. The transaction extends Canyon’s footprint across northern Colorado. The acquired company provides medical and non-medical home care, hospice, infusion services and medical equipment. Canyon Home Care & Hospice, founded seven years ago, serves a wide swath of communities across the northern portion of its home state, including communities such as Fort Collins, Loveland, Berthoud, Windsor, Greeley, Evans, Fort Morgan, Fort Lupton, Firestone, Frederick and Denver.
