CMS’ making care primary model launched July 1 with 133 participants representing 772 practices across the U.S. Participants in the model are based in eight states: Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina and Washington. The program, which runs through 2034, aims to improve care management and coordination, help primary care providers partner with specialists, and use community-based connections to address patients’ health needs and health-related social needs, such as housing and nutrition.
