Chapters Health System, the nation’s premier nonprofit chronic illness care organization, is pleased to announce the finalization of the affiliation with Willamette Vital Health, based in Salem, Oregon. This step marks the joining of two legacy nonprofit hospice organizations to expand access to critical chronic illness care for patients and families in the Pacific Northwest. Willamette Vital Health is the second organization to finalize its affiliation under the Chapters Health West division, which currently includes Nathan Adelson Hospice (Las Vegas, Nevada) and will also include Hospice East Bay (Pleasant Hill, CA) and Hospice of Santa Cruz County (Santa Cruz, CA).
